Item Description QTY. Unit 1 Purified Chick Embryo Cell Culture Rabies Vaccine 731 Vials 2 Equine Anti-Rabies Immunoglobulin 100 Vials TERMS AND CONDITIONS IN PURCHASING ANTI-RABIES VACCINE 1. Expiry Date All Anti-Rabies Vaccines and Rabies Immunoglobulin RIG, if applicable, must have a minimum remaining shelf life of two 2 years from the date of delivery. 2. Certificate of Supplier Verification The winning supplier shall submit a Certificate of Supplier Verification to be attached each delivery, together with the following supporting documents: a. Certificate/Authorization attesting that the supplier is authorized to sell the Anti-Rabies Vaccines and/or Rabies Immunoglobulin RIG, issued by the exclusive/authorized distributor, as reflected in the valid Certificate of Product Registration CPR Authorized Distributor to Local Distributor. b. FDA Certified True Copy with Dry Seal of the Certificate of Product Registration CPR for the offered Anti-Rabies Vaccine and/or Rabies Immunoglobulin RIG, to establish product registration and traceability Local Distributor to Recipient ABTC/ABTC. c. Batch Release Certificate 3. Product Registration All vaccines offered must be duly registered with the Philippine Food and Drug Administration FDA and must have a valid Certificate of Product Registration CPR at the time of bid submission and delivery. 4. Delivery Requirement The vaccines shall be delivered in accordance with the manufacturers recommended storage and transportation conditions, maintaining the required cold chain throughout delivery. Products found to have been improperly handled or compromised shall be rejected. 5. Replacement of Rejected Items Any vaccines delivered with damaged packaging, incomplete documentation, expired or near-expiry products, or those failing to meet the required specifications shall be replaced by the supplier at no additional cost to the procuring entity within the period specified in the Purchase Order or Contract.
